Mentoring Agreement
By submitting this form, I acknowledge that I have read, understood, and agree to the terms of this Mentoring Agreement. I understand that this agreement applies to all mentoring services provided by Karen Sarmento of Sarmento Mentoring Services LLC, including both individual and group coaching programs.
1. Services Provided
Karen Sarmento (“Mentor”) agrees to provide mentoring services designed to support personal and/or professional goals through regular virtual sessions, either individually or in a group setting. Mentoring is not therapy, medical care, or licensed healthcare treatment, and the Mentor does not diagnose, treat, or prescribe.
2. Scope of Relationship
This mentoring relationship is non-clinical and not a substitute for medical, psychological, or psychiatric care. The Client understands that mentoring is not regulated by the Board of Nursing or any healthcare licensing authority. By agreeing to this, the Client confirms they are not seeking clinical care through this mentoring relationship.
3. Confidentiality
All conversations and information shared in mentoring sessions will be treated as confidential and will not be disclosed without written consent, except as required by law (e.g., imminent risk of harm, child or elder abuse).
4. Commitment and Results
Mentoring is a collaborative process that requires the Client’s full participation. The Mentor makes no guarantees or warranties about results. The success of mentoring depends primarily on the Client’s commitment, effort, and follow-through.
5. Scheduling and Cancellation
- All sessions will be scheduled in advance, whether individual or group, via Zoom or another agreed-upon platform.
- A 24-hour cancellation notice is required for all individual sessions.
- If the Client cancels with less than 24 hours’ notice or does not show up, the session may be forfeited and considered used. The Mentor reserves the right to charge a cancellation fee equal to the session rate.
6. Payment Terms
- Mentoring fees are due in advance or as agreed upon at the time of enrollment.
- All payments are non-refundable unless otherwise stated.
7. Limitation of Liability
The Client agrees to hold harmless the Mentor and Sarmento Mentoring Services LLC for any decisions or actions taken as a result of mentoring. The Client understands the Mentor is not liable for any outcomes—positive or negative—resulting from the mentoring relationship.
8. Legal & Ethical Boundaries
The Mentor affirms she is a licensed Nurse Practitioner in Massachusetts but is not acting in that role during mentoring. The services provided are not under her NP license and should not be construed as such.
9. Termination
Either party may terminate this agreement at any time with written notice. Any outstanding fees remain due at the time of termination.

By submitting this form, I affirm that I have read and agree to the terms outlined above in this Mentoring Agreement.
